Job Description
Responsible for accurate coding of all pertinent diagnoses and procedures on outpatient medical records and for abstracting demographic and clinical information from the medical records, entering the data in the hospital computer system.
Requirements and Responsibilities
Bachelor’s Degree in Health Information System/ Management / Technology or Hospital Administration from an accredited university or equivalent with minimum of two (2) years in Medical Coding in acute healthcare / large hospital setting.
OR,
Two (2) years Diploma in Medical Records / Hospital Administration or equivalent from an accredited institute with minimum of four (4) years in Medical Coding in acute healthcare / large hospital setting.
Classified and Registered Health Information / Hospital Administration Specialist/Technician from SCFHS (Essential)
Registered Health Information Technician (RHIT), Certified coding Associate (CCA), Certified Coding specialist (CCS) or equivalent from an accredited organization (Desirable).

1. Accurately codes all established diagnoses and procedures for inpatient, outpatient and emergency records. |
|
2. Codes a minimum of: 130 outpatient records or 40 inpatient records. |
|
3. Provides all the diagnoses and procedures codes for research projects and physician’s studies in the absence of the Lead Coder/Analyst Trainer. |
|
4. Must codes all diagnoses/procedures at 95% accuracy. |
|
5. Performs all duties in a professional, effective, and confidential manner. |
|
6. Performs all other duties as required or assigned. |
